United Helpers, founded in 1898, is seeking ACT Case Manager with the skills, compassion, and commitment to deliver high-quality services to people in northern New York.
The Assertive Community Treatment (ACT) Team Case Manager provides or ensures the provision of services to program members in the most appropriate environment on a regular and emergency basis. Offers customer support skills building training, advocacy, linkage to available community resources and monitors general well-being and progress within service plans. Our successful candidate will possess a Bachelor’s or a Master’s Degree in Social Work, Psychology, Vocational Rehabilitation or a related Human Service field. One-year experience in case management is preferred. Must possess a valid New York State Driver’s License.
