Instructions for completing this interactive application:

If you have never completed an interactive form online please review the following help sheet before you do. (Help Sheet Link).

  1. First, click on the "Cover Sheet" link at the bottom of the page. Next click on the " Application" button at the end of this page.  An interactive PDF file should open for both of these links.  If you cannot fill in the application Stop and go to #5. 
  1. Fill in all areas and sections of the application.
  1. After filling out the application electronically sign as indicated at the end of the application under the signature line

 

NOTE: Incomplete or unsigned applications will be returned. 

  1. Once the application is electronically signed you will then be able to submit it by e-mail.  If you use MS Outlook or MS Outlook Express you can click the submit button at the bottom of the application form.  This should start your default e-mail software mentioned above. 

 

If you use any other e-mail software (i.e. Yahoo, AOL, Google, Hotmail etc.) you will need to save a copy of the application PDF file to your hard drive and then send it as an attachment using your e-mail program to application@unitedhelpers.org

  1. If you cannot fill-in this application online, print one out and complete by hand. 

 

To print a blank copy of the application, open up the file and at the bottom of the form you will see a print button.  Click on the button select the printer if necessary click OK.

Fill out all sections of the application.  SIGN the application and mail to:
