Unlike traditional outpatient services, our mobile Assertive Community Treatment (ACT) team serves patients where they live and work. ACT provides case management, life skills development, and support to assist individuals with increased opportunities for independent living.

A Mobile Team That's Never Far Away
ACT provides services for people living with chronic and persistent mental illness. The Team serves St. Lawrence County residents to ensure continuity of care from a hospital to an outpatient practice, improve patients’ ability to function in the community, and reduce the need for future hospitalization.
