Safely at home
You’ve lived independently in the home where you raised your family. Now get the care you need to stay there. United Helpers, in concert with Canton-Potsdam Hospital and Claxton-Hepburn Medical Center, recommends Northern Lights Home Health Care, part-time, intermittent health care, and support services for St. Lawrence County residents that need intermediate and skilled health care.
Given the choice, many people would opt to stay in their own homes. Northern Lights Home Health Care makes that possible, for as long as possible. Sometimes, loved ones simply need a friendly companion to help with everyday activities. Other times, they need a more specialized care plan, which we can coordinate among their family, physician, and anyone else involved in their well-being.
Home Health Services are provided by Northern Lights Home Health Care. Northern Lights Home Health Care is a consortium of Canton-Potsdam Hospital, Claxton-Hepburn Medical Center, and United Helpers Management Company, three of the most trusted health care agencies in the north country.
A respected care team
Our care begins with an assessment from a Case Manager who will develop a plan of care specific to each patient’s needs. The care plan is implemented only after approval of the patient’s own physician, who is kept informed of the patient’s progress throughout the time he/she is under our care.
Each member of our staff is hired with compassion and empathy in mind. All of our employees undergo rigorous background checks, receive additional training, and are required to attend ongoing in-service education throughout the year.
Proudly offering the following services in the comfort of your own home:
- Skilled nursing
- Physical therapy
- Speech therapy
- Occupational therapy
- Nutritional therapy
- Social work
- Personal care
A physician’s order must be provided before we may evaluate the patient’s need for home care services. Paying privately is also an option.
