Supportive Housing

Helping to provide financial assistance in order to secure safe, affordable housing.  Supportive housing and Rental Assistance Programs offer rental stipends and case management for individuals with serious and persistent behavioral health conditions.

Assistance For Safe & Affordable Solutions

Supportive Housing Case Managers help individuals enrolled ensure basic needs are met and maintained. Case Managers travel all throughout St. Lawrence County meeting clients at their homes ensuring independence. If you are interested in joining our team, please reach out to our Navigator Line at (315) 714-3117 to get more information about the program. If you are a provider and think Supportive Housing Case Management services would benefit a patient, please complete a SPOA referral.
